Get your Google Form Embed Code
Log in to Google Forms to open your Form, then follow the instructions below:
When viewing your form, click the Send button on the top right.
Click the <> tab option to access the embed code.
Copy the Embed HTML and proceed to the next steps.
Add a Form Embed Section to your site
In the Siimple Site Editor for your site, click on Sections at the top and add the Form Embed Section.
Select Google Forms for the Form Service then click the Add your embed code button.
In the popup that appears, paste the embed code you copied from Step 1.
Save and publish your site to see your form live!
⚠️ Embedded Google Forms will not appear in preview mode — they will only display on your live published site. Be sure to publish the page before checking the form.
